Answer to Question 1

In the MySQL Workbench, new database designs are created using either the File  New Model command or the New Model button. MySQL calls these models EER models, but they are actually database designs. At this point, the actual ER diagram is created by double clicking the Add Diagram icon in the Model Overview section on the MySQL Model page.
