Answer to Question 1

Release the affected wrist. Allow the patient to do ROM and see if the numbness and tingling goes away. Replace the restraint following manufacturer's instructions and make sure the restraint is not too tight. A finger should easily fit between the restraint and the wrist. Notify the nurse immediately.

Answer to Question 2

Eyeglasses need to be worn to help prevent falls. Clothing needs to fit properly and not drag on the ground. Non-skid footwear should be worn. Either non-skid socks or properly fitting socks with well-fitting shoes properly laced or fastened should be worn. Throw rugs should not be used, and varied colors and designs can cause balance problems and dizziness in older persons.
