Rationale: The PA axial Caldwell requires OML to be aligned perpendicular. With a 15 degree caudal angle, this will push the petrous pyramids into the lower one-third of the orbits.

extension lateral

Rationale: Extension lateral will require the patient to slowly raise the chin, extending the chin until maximum tolerable position is attained. The spinous processes will come closer together. Flexion and extension laterals will evaluate the mobility of the neck.
