Answer to Question 1

A
Nocturia, or urination during the night, disrupts sleep and the sleep cycle. This condition is most common in older adults with reduced bladder tone or persons with cardiac disease, diabetes, urethritis, or prostatic disease. After a person awakens repeatedly to urinate, returning to sleep is difficult. Although all the clients may have difficulty falling back to sleep when awakened, this answer represents the client with the greatest tendency to be awakened during the night.
A 15-year-old with anxiety does not represent the client having the most difficulty returning to sleep.
A 35-year-old with hypothyroidism does not represent the client having the most difficulty re-turning to sleep.
A 55-year-old with hypertension does not represent the client having the most difficulty return-ing to sleep.

Answer to Question 2

C
By 9 months, infants play simple social games such as patty-cake and peek-a-boo. By 1 year, infants not only recognize their own names but are able to say three to five words and understand almost 100 words. Extended separations from parents complicate the attachment process and increase the number of caregivers with whom they must interact. Ideally, the parents provide most of the care during hospitalizations.
