The best test to differentiate sickle cell trait from anemia is hemoglobin electrophoresis. Sickle cell trait should have roughly 40 Hb S and 55 Hb A, whereas sickle cell anemia should have predominantly Hb S with a variably increased Hb F and no Hb A.
