Answer to Question 1

C
First square every score, and then add the squared values. When told to multiply (square) and add, but given no parentheses, multiply first and then add.

Answer to Question 2

D
There is no Mean Square Total value in an ANOVA summary table, but for the sums of squares and the degrees of freedom both, the within and between values must equal the total value.
