An expected outcome is a description of the favorable patient status that is achieved when a prob-lem-focused nursing diagnosis has been resolved or positive adaptation has been enhanced. Clearly written outcome statements stipulate the patient's expected behavior or function within a specified time frame, rather than what the nursing interventions will accomplish. Some outcome statements require qualifiers or the conditions involved with the patient's activity.

The wound should be irrigated with normal saline prior to culturing because it decreases the risk of culturing normal flora. Irrigating with an antiseptic prior to culturing may destroy the bacteria. The goal is to identify bacteria that are invading the tissues so slough or eschar should be avoided. The area of the wound with the most exudate should be cultured, near the center of the wound and not the edges.
