Medication orders must also be legible. If the nurse is unsure of any part of the order, she or he must seek clarification from the prescribing provider, and not make an assumption that the provider meant to order drug X, not drug Y.

For example, the dosage of a drug that is appropriate for a young or middle adult may be harmful to an older adult whose liver and kidney function is less efficient.
