Answer to Question 1

D
Medication orders must also be legible. If the nurse is unsure of any part of the order, she or he must seek clarification from the prescribing provider, and not make an assumption that the provider meant to order drug X, not drug Y.

Answer to Question 2

C
Individual client characteristics such as age, weight, height, and physical and mental conditions can greatly influence drug action. For example, the dosage of a drug that is appropriate for a young or middle adult may be harmful to an older adult whose liver and kidney function is less efficient.
