Answer to Question 1

Meet the kcal needs according to size, activity, weight gain, and disease state if present; i.e., infection, fever, etc. Protein may increase to 1.0 g to 1.5 g/kg IBW depending on the conditions as mentioned above. Multiple vitamin and mineral supplement daily with additional increases as conditions warrant.

Answer to Question 2

The GTT that was formerly used to diagnose diabetes is still used, but it is not recommended to be used routinely. The old values were:
 90 min < 200 mg/dl
 2 hr < 140 mg/dl
 3 hr < 125 mg/dl

Currently, the American Diabetes Association recommends the following criteria be used:
 2-h postload glucose < 140 mg/dl (7.8 mmol/l) = normal glucose tolerance.
 2-h postload glucose 140199 mg/dl (7.811.1 mmol/l) = IGT (impaired glucose tolerance).
 2-h postload glucose 200 mg/dl (11.1 mmol/l) = provisional diagnosis of diabetes (the diagnosis must be confirmed, as described below).
