Answer to Question 1

1
The nurse and client together measure how well the client has achieved the goals for health maintenance in the plan of care. Goals that have not been achieved are evaluated and adjusted. This is an activity done during the evaluation step of the nursing process. Evaluation of goals is not done during the assessment, planning, or implementation steps of the nursing process.

Answer to Question 2

2
Anticipatory grieving is the intellectual and emotional responses and behaviors by which individuals work through the process of modifying self-concept based on the perception of potential loss. Anger and crying about the client's pending death are signs of Anticipatory Grieving. The other assessment findings are evidence that the spouse is accepting the caregiver role.
