Endometrial cysts are caused by endometrial overgrowth and are often filled with old blood, which leads to the name chocolate cysts. Endometrial cysts are the result of endometrial implants on the ovary and are associated with endometriosis. These cysts are not associated with pelvic inflammatory disease, use of oral contraceptives, or hormone imbalance.

Leuprolide acetate (Lupron) is used to reduce bleeding and shrinks the fibroids. The use of oral contraceptives may reduce heavy bleeding. Medications that stop ovarian production of estrogen and progesterone (hormones necessary for fibroid growth) may provide a temporary decrease in manifestations. Iron replacement therapy is not appropriate for this patient.
