Patients with aortic dissection are initially treated with intravenous beta-blockers such as esmolol (Brevibloc) to reduce the heart rate to about 60 bpm. Sodium nitroprusside (Nipride) infusion is started concurrently to reduce the systolic pressure to 120 mmHg or less. Calcium channel blockers such as verapamil (Isoptin) or diltiazem (Cardizem) also may be used. Direct vasodilators such as hydralazine (Apresoline) are avoided because they may actually worsen the dissection.
