1. Altered sleep patterns might be the reason for starting the substance abuse.
2. The nurse should assess the client's ability to function on a daily basis and to what level the abuse has affected performance.
3. Since the client is seeking health care the nurse should not confront the client about the abuse in this manner. A statement like this would not build a trusting relationship.
4. Headaches can occur during withdrawal, or could be the reason for the abuse.
