Answer to Question 1

Answer: 2

1. Self-confidence and a positive attitude are not delusional behaviors.
2. The client is confident with a positive attitude, which promotes mental health.
3. Bragging might be exhibited by the client's saying that the job was in the bag or that no one else could top the client. This client is simply confident.
4. A depressed client is usually not confident with a positive attitude.

Answer to Question 2

Answer: 1

1. Exhaustion is a risk factor for mental illness, and good sleep patterns promote mental health. The nurse inquires about what is a normal sleep pattern for the client.
2. Since the client brought up the sleep disturbance, the nurse explores that avenue, not asking about a home.
3. Obviously, the client lives with a child. Sleep patterns are the first concern, since the client seems tired.
4. The client's financial state is not important at this time.
