The nurse is administering insulin to a client with type I diabetes mellitus and alternates the sites of the injections because:
 
  1. increased bleeding can occur.
  2. using the same site is painful.
  3. lipoatrophy can occur.
  4. absorption is too rapid when using the same site repeatedly.

Question 2

A client with diabetes mellitus can effectively monitor the blood sugar at home. The nurse should encourage the client with type 1 diabetes mellitus to monitor:
 
  1. indirect blood glucose monitoring.
  2. direct blood glucose monitoring and urine testing for glucose and ketones.
  3. hemoglobin A1c testing.
  4. home cholesterol testing.

2. Generally, insulin injections are not too painful unless the insulin is cold.
3. Lipoatrophy can occur with use of the same site, which would eventually affect absorption.
4. Absorption can be decreased when using the same site.

1. There is no indirect blood glucose monitoring except maybe urine testing for glucose. The client with type 1 diabetes mellitus would utilize this but also needs to use direct blood glucose monitoring.
2. Direct blood monitoring allows the client to monitor high and low values of blood glucose. Urine testing is useful for the client with a high blood glucose level to determine whether the client is experiencing ketoacidosis. The client with type 1 diabetes mellitus monitors both tests at home.
3. Hemoglobin A1c is done by the lab, not at home.
4. There is no home testing for cholesterol.
