2. The reduction of hormones changes the secondary sex characteristics.
3. The decrease of vaginal secretions causes a decrease of lubricant, which can cause intercourse to be uncomfortable.
4. Fibrosis in the ducts of the breasts has nothing to do with painful intercourse.

1. The umbilical cord would only protrude in the pregnant client.
2. The nurse is inspecting for a bulging vaginal wall, protrusion of the cervix or uterus, and urinary incontinence.
3. The nurse inspects the anus for tears, but not by asking the client to bear down.
4. The purpose of asking the client to bear down is not looking for stool incontinence.
