Answer to Question 1

Answer: 2

1. Urgency is a sign of overactive detrusor muscle, and there is no hematuria.
2. One of the first signs of cancer of the bladder is painless hematuria. The nurse needs to ask the client whether there is any pain when the client has hematuria.
3. Getting up at night is seen in clients with hyperglycemia and clients with urinary retention.
4. Stress incontinence does not involve hematuria, and is due to malfunction of parts of the urinary tract.

Answer to Question 2

Answer: 3

1. Antihistamines are not contraindicated with Gantrisin.
2. The nurse might want to check with the doctor if other antibiotics are being taken, but it would not be a big concern for the nurse.
3. The sulfonamides can cause bleeding. Any client taking nonsteroidal anti-inflammatory drugs (NSAIDs), aspirin, or blood-thinning agents should not take this antibiotic.
4. Gantrisin should be taken on an empty stomach, but antacids could be taken 2 hours after administration of the Gantrisin.

Although the Roman numeral for the number 4 has always been taught to have been "IV," according to historians, the ancient Romans probably used "IIII" most of the time. This is partially backed up by the fact that early grandfather clocks displayed IIII for the number 4 instead of IV. Early clockmakers apparently thought that the IIII balanced out the VIII (used for the number 8) on the clock face and that it just looked better.
