The nurse is caring for an older client in the home and notes that the client is very intelligent but has a tendency to experience short-term memory loss. What should the nurse do to help the client with this memory change?
 
  1. Suggest that the client live with a younger adult.
  2. Prepare the client for living in a long-term care facility.
  3. Plan memory aids with the client, such as writing appointments on a calendar.
  4. Tell the client to have people call to remind the client of upcoming appointments.

1. Short-term memory loss does not mean that the client should live with a younger family member who has a good memory.
2. The client with short-term memory loss is not a candidate for a long-term care facility.
3. Short-term memory loss can be addressed by the nurse and client by suggesting new ways to prompt the client's memory.
4. Some physicians will automatically call with reminders of appointments, but it is better to help the client establish ways to remember that foster independence.

1. There is no proof that the elderly lose intelligence as they age.
2. The elderly do not continue to grow and develop physically.
3. The greatest growth is seen during childhood. The younger adult does continue to grow and develop physically, but not at an accelerated rate.
4. The elderly do have developmental tasks to complete.
