Answer to Question 1

Answer: B

For clients who are at risk of overstimulation, nurses should assist with reducing the number and types of environmental stimuli. The nurse can counteract sensory overload by blocking stimuli and by helping the client organize the stimuli and alter responses to the stimuli. Dark glasses with ultraviolet (UVA and UVB) light protection can partially block light rays. It would be impractical for a person to avoid going out of doors during the day to prevent visual stimulation. The client has glaucoma and should not be using artificial tears or any over-the-counter medication without first discussing it with their healthcare provider.

Answer to Question 2

Answer: A

Clients who are considering pregnancy should be advised of the importance of testing for rubella, which can cause vision impairments in newborns. Zinc is not known to prevent sensory impairments in newborns. Limiting vitamin A and avoiding foods high in folic acid could lead to developmental deficits in the newborn and should not be recommended to the client.
