The appropriate action by the nurse is to assess the client for a source of the infection. Instructing the client on the use of an electric razor and soft toothbrush and assessing for bleeding and bruising would be appropriate actions for a client with decreased platelet levels, or thrombocytopenia. Placing the client in reverse isolation precautions would be appropriate for the client with neutropenia, a decrease in the number of neutrophils.
