The nurse is attempting to withdraw 1 mL of fluid into a 3 mL syringe, and is pulling on the plunger. The fluid is not flowing into the syringe, and the nurse has withdrawn the plunger to the 2.5 mL line.

Throwing the vial away is unnecessary, and the nurse is likely to find the same problem when trying the next vial. A larger syringe is not the best option for correcting the problem, because the vacuum within the vial will still exist.
