Answer to Question 1

Correct Answer: 1

It is appropriate to monitor an infant's temperature using the axillae. It is not appropriate to use the temporal artery, the tympanic membrane, or the oral cavity to assess an infant's temperature.

Answer to Question 2

Correct Answer: 4

If a client is resting and stable, there is no need to measure vital signs until the client is awake. It is appropriate to measure vital signs before and after a procedure, on admission to the hospital, and when the client is in pain.
