Anesthesia for cardiac catheterization including coronary angiography and ventriculography (not to include Swan-Ganz catheter). The patient has severe systemic disease due to coronary artery disease. An anesthesiologist provided anesthesia. Assign the CPT anesthesia code with appropriate CPT physical status and HCPCS level II anesthesiologist modifiers.
