In cases of communicable diseases, the physician or health care facility may be required to report to the department of health the number of cases and:
 
  a. other demographics that assist in preventing the spread of communicable diseases.
  b. names of and other medical information regarding the patients with the disease.
  c. lists of people the patient has come in contact with in the last 24 hours.
  d. the treatment provided to the patient.
