Vickie, 37 years old, was recently diagnosed with relapsing/remitting MS. She is seeing an ophthalmologist because of problems with her vision. She has had some episodes of double vision and wants to find out if it is related to the MS. After completing a thorough eye exam and taking a detailed medical history, Vickie is diagnosed with bilateral internuclear ophtalmoplegia. The ophthalmologist said the condition is directly related to the MS and told Vickie to continue on her present treatment regimen. She expects the double vision will fluctuate along with the other MS symptoms.

Tom is a 42-year-old black male who presents to the ED with vision problems. He states that when he woke up this morning, he noticed that the vision in his left eye was funky. He describes it like having a dark shadow across the lower portion of my eye. He had no visual disturbances when he retired the night before, and denies any vision problems previously. He does not wear glasses for distance, though he acknowledged he would probably need reading glasses soon. He denies any history of hypertension, diabetes or high cholesterol. He is very physically fit and says he watches his diet and exercises religiously. A neuro-ophthalmologist was called to consult and she diagnosed Tom with ischemic optic neuropathy. She prescribed large doses of steroids and told Tom there was a good chance his vision would improve or return entirely. She told Tom to follow-up with her in one week.
