 The site of service is the location in which an E/M service visit occurs, and each site presents a different set of parameters, requirements, and circumstances that influence the E/M service visit.
 The CPT book provides an exhaustive list of numerical codes that represent the many different categories of E/M service visits; each category of codes represents a different site of service.
 The patient type defines whether the patient has received clinical, medical, or professional services from the performing physician, or another physician of the exact same specialty and subspecialty who also belongs to the same group practice, during three years prior to the E/M service in question.
 New patients have not received such services and require that all three of the key components be considered to determine the level of E/M service.
 Established patients have received such services and require only that two of the three key components be met or exceeded for the E/M service to qualify for a particular level
