Answer to Question 1

The long answer should include following:
 Eight HPI elements: Answers should be similar to (but not copied from) the examples listed in the core textbook
 Chief complaint: Answer should be similar to (but not copied from) the example listed in the core textbook
 Signs and symptoms: Answer should describe a physical experience of discomfort, feeling unwell, uncomfortable, or out-of-sorts, such as a headache, stuffy head, and watery eyes experienced during hay fever season.

Answer to Question 2

 The performing physician must collect and document the history of present illness because the performing physician may notice something in the patient's manner that may provide additional information or insight into the CC.
 The importance of this stems from the clinical knowledge, experience, training, and expertise of the performing physician in comparison with any other clinical staff member. And, since the performing physician will also be responsible for assessing and treating the patient, it is important that the performing physician understand the personal nature of this CC.
