Answer: An encounter form, also called a superbill, is the form used to document information about a patient visit or encounter, including the diagnosis made by the physician and the services rendered. Information on this form is used in generating bills and insurance claims.

Answer: Any of the following: Patient name, patient address, telephone number, insurance company, insurance address, type, group and policy number(s), and policyholder, employer name and address, emergency contact information, and the signature authorizing the release of information for claims processing and the assignment of benefits.
