   2. Assessing the country of origin.
   3. Assessing needs that are verbalized by the patient.
   4. Assessing spiritual and cultural aspects of care and including the patient and caregivers in care planning.

A preschooler that is being potty trained is refusing to have bowel movements. The mother brought the child to the clinic because now the child has constipation.
 
  The nurse speaks to the mother about behavior modification efforts to have the preschooler defecate. All of the following may help except: 1. Rewarding the child for defecating in the toilet.

4. Reverting back to diapers may make the child refuse to use a toilet because of the convenience.
