The Institute of Medicine issued two reports (2000, 2001) which laid out strategies to reduce medical errors. The reports were directed toward government, industry, consumers, and health care providers. The strategy was initiated because of reports of death and injury as a result of medical errors. These reports led to the formation of the National Patient Safety Foundation by the American Medical Association and a reporting system by the Joint Commission (formerly known as the Joint Commission on Accreditation of Healthcare Organizations) (The Joint Commission, 2010).
