At an industrial complex, a young male was struck in the head by a large piece of steel thrown from a grinding machine. Assessment reveals a soft, painful depression underlying a tear in his scalp, which is covered with dried blood. There is also clear fluid coming from his right ear. He is conscious but confused and exhibits the following vital signs: pulse 84, respirations 16, blood pressure 142/76, and SpO2 92.
