Answer to Question 1

B, C, D, E
When working to sustain evidence-based practice, strategies are needed to hardwire the work into the organizational system. One strategy to promote integration of an evidence-based practice is to incorporate the evidence-based practice into organizational policies and procedures. Linking quality improvement and evidence-based practice is another strategy for building a strong orga-nizational context. Successes need to be rewarded along the way. Celebrations help build a cul-ture that supports and expects the use of evidence in practice. Rewards should include formal recognition from high-level organizational leaders, visibility for the team and project champions, accessibility to practitioners within the organization, and a clear articulation of the benefits of evidence-based practice.

Answer to Question 2

B, D
It is possible that the nurse and nurse manager could be held negligent. The nurse could be held negligent for not moving the patient closer to the nurse's station, calling the physician or nurse practitioner for medications or change in medications, or obtaining an order for a vest restraint. The nurse manager may be held negligent because of 24-hour accountability for the care of pa-tients on his or her unit and possible lack of supervision.

Certain triggers may cause a drug abuser to relapse. Research shows that long-term drug abuse results in significant changes in brain function that persist long after an individual stops using drugs. It is most important to realize that the same is true of not just illegal substances but alcohol and tobacco as well.

People who have myopia, or nearsightedness, are not able to see objects at a distance but only up close. It occurs when the cornea is either curved too steeply, the eye is too long, or both. This condition is progressive and worsens with time. More than 100 million people in the United States are nearsighted, but only 20% of those are born with the condition. Diet, eye exercise, drug therapy, and corrective lenses can all help manage nearsightedness.
