Answer to Question 1

Correct Answer: 1

In viewing patients holistically, nurses must consider the spiritual and religious beliefs of patients when providing care. Nurses should understand how spiritual beliefs influence the health and decision making of patients. Physical needs are part of holistic care but do not encompass the totality to the patient's needs. The degree of patient illness does not determine the need for holistic nursing care. Hospital chaplains may assist with spiritual needs of the patient but the nurse retains responsibility for spiritual assessment and holistic nursing care.

Answer to Question 2

Correct Answer: 3

Spiritual distress, a nursing diagnosis, is the state in which a person or group experiences a disturbance in the belief or value system that provides strength, hope, and meaning to life. Spiritual distress may be the cause of illness, especially with emotional distress; or it may occur as a result of illness as one questions why a situation is occurring in his or her life: Why would God let this happen to me? Impaired religiosity is the state in which a person or group has an impaired ability to exercise reliance on beliefs or a particular denomination or faith community, and to participate in related rituals. Spiritual awakening and impaired belief are not appropriately written nursing diagnoses.
