Answer to Question 1

C
The state mandates the rules about having immunizations that the nurse must follow. Thus, it is the responsibility of the school nurse to be aware of the laws in the state regarding acceptable reasons for immunization exemption.

Answer to Question 2

C
A current, signed parental consent form for administering the medication is essential for the stu-dent's file. The prescribed drug must have the original prescription label on it and be in the orig-inal container so that there are no errors. It is recommended that the physician contact the school nurse or vice versa, but a signed order from the physician is not necessary.
