Answer to Question 1

B
Feedback:
Perhaps the most important skill for nurses to use in reminiscing activities is listening. Little encouragement or guidance is often needed for elderly people to reminisce, and redirecting is only important if the patient starts to ramble aimlessly.

Answer to Question 2

C
A lumbar puncture is contraindicated in patients who might have increased intracranial pressure, because the withdrawal of fluid may cause the medulla oblongata to herniate downward into the foramen magnum.
