Answer to Question 1

D
Restraining (response A) or medicating (response C) would not increase his nutritional intake. Offering more choices (response B) may or may not be beneficial. Carrying his food and drink with him adjusts his meals to accommodate for his behaviors and probably would be effective.

Answer to Question 2

B
The nurse must ensure that the advance directive reflects the wishes of the resident. The other choices are inappropriate because they are outside the scope of the licensed practical nurse.
