Answer to Question 1

C
Feedback:
Many older clients are reluctant to admit they are depressed. The negative stigma attached to psychiatric diagnoses is different for individuals born in the 20th century than citizens of this century. Depression is not a normal part of aging; however, it is very common because the elderly have increased risk factors. Cumulative losses and numerous changes, most of which are beyond a person's control, can lead to depression. Clinical depression is often underdiagnosed or misdiagnosed in older adults.

Answer to Question 2

A
Feedback:
The nurse should caution the client to swallow only very small amounts of the swish-and-swallow solution at a time because the mixture will anesthetize the throat and may cause difficulty in swallowing, talking, or even breathing. Brushing teeth, using mouthwash, and flossing are not recommended following this treatment. With stomatitis, teeth should be brushed gently and flossing should not be done.
