Grading is a system of looking at abnormal cells under a microscope to determine the cells' degree of dedifferentiation or lack of maturity. Generally, a system of three to five grades is used. Grade I tumors closely resemble normal cells or well-differentiated cells. They retain much of their differentiation as specific tissue cells. Grade II or III tumors have intermediate phases of differentiation, ranging from slightly undifferentiated to extremely abnormal cells. Grade IV or V tumors are very anaplastic or poorly differentiated, often having little or no resemblance to the tissue cells from which they developed. Cancer in situ refers to tumor cells that have not invaded surrounding sites.
