Answer to Question 1

D

Answer to Question 2

C
Feedback:
In most facilities, the night nurse performs routine once-a-day record-keeping at midnight. Clients who are continent in the daytime may be incontinent at night; it is important to check them frequently. Some medications, such as sedating psychotropic medications, laxatives, and vaginal suppositories, are routinely given in the evening. Clients are more likely to fall when it is darker and/or they are more confused.

It is tightly attached to the skin below it, so it compensates for the increased area by wrinkling. This happens to the hands and feet because they have the thickest layer of dead keratin cells.
