Answer to Question 1

A

Answer to Question 2

A
Feedback:
The nurse should report to the practitioner if the client feels the urge to void but is unable to do so. Other measures, such as catheterization or suprapubic pressure, should not be performed unless advised by the practitioner. Failure to void may indicate swelling or injury to the urinary system; therefore, reassurance is not sufficient.
