Answer to Question 1

B
Feedback:
The nurse should help the client with sitz baths to ease voiding and to soothe the affected area. Because the fluid medium is infused into the bladder at high pressures, infected urine can be forced up into the kidney, causing kidney infection. Therefore, it is important to obtain a urinalysis and urine culture before this procedure to determine if a client has a kidney infection. Nursing considerations also include reporting blood-tinged urine for more than 24 hours and encouraging the client to drink fluids to prevent urinary stasis and flush any remaining dye from the body.

Answer to Question 2

C
Feedback:
The greatest fear of most clients is that they will be left alone to die. Caregivers must be alert to this fear and provide more support as illness progresses. Volunteers can assist by staying with clients so they are not alone. The second greatest fear of most clients is that of having uncontrolled pain. Pain can usually be managed successfully.
