Initially, nurses triage all clients to determine clinical acuity and the need to be seen immediately. In some states, initial triage must be performed by an RN. Triage by telephone is known as teletriage. Risk assessments, client identification, and client interviews would be performed by the nurse following triage.

Thyroid scan (radioscan or scintiscan) is used. For this test, the client ingests radioactive iodine or receives it intravenously (IV). Then a scanogram (x-ray study) is obtained to indicate the amount of radioactivity in the entire body. If the thyroid absorbs a great deal of the radioactive iodine, the thyroid is hyperactive, suggesting possible hyperthyroidism. Thyroid suppression test measures levels of radioactive iodine uptake and serum T4 after 7 to 10 days of receiving thyroid hormone. TSH stimulation test measures body's response to TSH by following levels of radioactive iodine uptake and protein-bound iodine uptake. TRH stimulation test measures TSH levels repeatedly after thyroid-releasing hormone (normally produced by the hypothalamus to stimulate TSH) is given IV.
