Answer to Question 1

D

Answer to Question 2

A
Feedback:
The nurse should position the chair at a 45-degree angle to the bed for easy transfer of the client to the chair. The nurse should not ask the client to hold the arms of the chair for support. The arms of the chair are kept out of the way to prevent the client from getting caught in it. The nurse should not move the client into the chair by pulling the client's waist. The nurse should seek the assistance of another nurse; as one nurse holds the client under the axillae from behind, the other nurse faces the wheelchair and supports the client's knees and legs. The bed must not be raised to a level higher than that of the chair.
