The Five Rights of medication administration are the right client, right medication, right dose, right time, and right route. The sixth right is right documentation, and seventh right programming when using pump administration.

The nurse should report shallow, rapid breathing to the physician when observing symptoms of internal bleeding in a client. A client with internal bleeding may also show extreme thirst, falling BP, or rapid pulse. A client with internal bleeding does not show extreme hunger, slow pulse, or high BP.
