Answer to Question 1

B
Feedback:
Sitting upright or in a semisitting position helps the client to alleviate the condition. Lying supine with a single pillow or lying supine with head slightly lowered does not help the client to relieve the condition. Lying with the legs slightly elevated helps in improving the circulation of the limbs, but does not relieve breathing difficulty.

Answer to Question 2

D
Feedback:
When caring for a client with pneumothorax, the nurse should seal the client's wound using petroleum jelly gauze. The nurse may also use aluminum foil, plastic wrap, or rolled-up dressing to seal the client's wound. Lifesaving measures, such as CPR, are required in the case of near drowning. Checking for signs of inhalation injuries is required for burn cases. Placing ice packs on the forehead is required for treating heatstroke.
