Answer to Question 1

D
Feedback:
The nurse should remind clients to wiggle the fingers or toes of the affected extremities. Too little or too much exercise can be harmful. Clients should be encouraged to exercise unaffected body parts as much as possible and flex and extend unaffected joints regularly. The nurse can place a trapeze on the bed frame of clients who are confined to bed so they can help lift their bodies for nursing care and use the trapeze for exercise.

Answer to Question 2

D
Feedback:
A rare, but serious, complication of thoracentesis is pneumothorax, which is an accumulation of air in the pleural space. Hypotension and pulmonary embolism are not side effects of this procedure. The procedure is performed to remove excess fluid from the pleural cavity.
