Feedback:
The nurse should diagnose this condition as disturbed thought process. Disturbed thought process is related to HIV-related encephalopathy, suicidal ideation, depression, and anxiety. Impaired tissue integrity is diagnosed in relation to thrush, shingles, athlete's foot, other fungal infections, altered nutritional status, and suicide attempts. Ineffective breathing pattern is associated with Pneumocystis carinii pneumonia and tuberculosis infection. Risk of disuse syndrome is diagnosed in relation to immobility, fatigue, and decreased level of endurance.
