Answer to Question 1

C
Feedback:
Move the client into a side-lying position, not prone, Sims', or supine with the head of the bed lowered, as tolerated. The client's head may also be turned to the side. In this position, gravity causes saliva to run out of the mouth and prevents the client from choking. This position also allows for the client to be suctioned.

Answer to Question 2

B
Feedback:
The nurse should encourage the client with contact dermatitis to pat the area lightly because doing so controls itching and prevents further irritation and skin breakdown. Cool compresses, not warm compresses, control itching and keep the area clean. The nurse should caution the client not to use topical corticosteroids hourly because overusing the ointments could lead to systemic absorption. The nurse should not bandage and keep the area covered. The nurse should encourage the client to keep the area clean and dry, and avoid vigorous rubbing.
