Answer to Question 1

A
Feedback:
The nurse would assume that the wound is contaminated and follow standard procedures. Existing dressing may be premoistened with warmed sterile, normal saline, if ordered. A bed cradle should be used when applying packs to the client's legs or torso to prevent wicking of the solutions to the linens. Also, the client should be medicated with analgesics and anxiolytics (antianxiety)  to 1 hour before the procedure.

Answer to Question 2

A
Feedback:
The nurse should not use a trailing zero after a decimal point (when dosage is expressed as a whole number) or leave a naked decimal point. For example, if the dosage is 1/4 of a milligram, it should be written as 0.25 mg.
