Answer to Question 1

D
Feedback:
The nurse can develop a rapport and effectively communicate with the client by addressing the client as Mr. followed by his last name. The nurse should show respect for the client's seniority and self-esteem. The nurse should not refer to the client as Grandpa, because it is disrespectful and offends the client. The nurse does not need to be firm when instructing the client, nor should the nurse change the subject when the client indicates disinterest in the treatment because these will not help in developing a rapport or communicating effectively with the client.

Answer to Question 2

D
Feedback:
Indicating to the client an understanding of the client's discomfort demonstrates concern for the client. Explaining the procedure for dressing the wound and informing the client of actions to be taken helps the client to participate in the nursing care. Telling the client that he or she will recover soon would not be appropriate because it could be offering empty reassurances.
